| These services can be provided in the home, hospital, nursing home, or assisted living facility. |
| We will meet with you in your home to develop with you a plan of care that best meets your needs. This plan of care will be revised as needed, and it will be evaluated monthly for any areas needing improvement with a home visit by a member of our management team. |
| The cost for our services is based on an hourly rate, so your hours can be increased or decreased as your needs change. Our services are available from as little as 3 hours a day to 24 hours a day, for short or long-term care. |
